Pathways Hospice Foundation

The Pathways Hospice Foundation is a non-profit organization that was founded to provide our patients and families with special non-medical needs or final wishes that can arise at this most difficult and vulnerable time.

Your gift to the Pathways Hospice Foundation will help us provide comfort and compassionate care to our patients and families during their loved one's final months and days of life, allowing them to make every moment count.

How Donations Are Used

100% of funds are used to support our patients and families in need. Foundation funds have recently been used to:

• Cover transportation costs that enable family members who otherwise cannot afford it.

• Assist with paying for final arrangements/funeral expenses for families who otherwise cannot afford it.

• Pay utility bills to enable patients to stay at home.

• Provide assistance with day-to-day needs or emergencies.

• Fulfill special last wishes.
