Honoring Excellence in Hospice Care
CAHPS Survey Recognition
At Pathways Hospice & Palliative Care, our mission is to provide compassionate, personalized support to patients and their loved ones during life’s most delicate transitions. One of the ways we measure the quality of our care is through feedback from the families we serve — and we’re honored to be recognized nationally for that commitment.

What is the CAHPS Hospice Survey?
The CAHPS Hospice Survey (Consumer Assessment of Healthcare Providers and Systems) is a national program overseen by the Centers for Medicare & Medicaid Services (CMS). After a hospice patient passes away, a confidential survey is mailed to their primary caregiver or loved one, giving them the opportunity to share their experience with the care their loved one received.
This feedback is essential. It helps ensure transparency, encourages accountability, and guides ongoing improvements in hospice care across the country — including here at Pathways.

Recognized for Excellence
We are proud to share that Pathways Hospice was the recipient of the:
-
2023 CAHPS Honors Elite Award – Recognizing top-tier performance and family satisfaction among hospice providers nationwide.
-
2025 CAHPS Honors Award – Awarded to agencies that are recognized for exceeding the national score averages across nearly all areas of care.
These honors reflect the dedication and heart of our entire team — from nurses and aides to administrative staff and volunteers — and serve as a reminder that our work makes a meaningful difference during some of life’s most important moments.

Your Voice Matters
If you receive a CAHPS Hospice Survey after the passing of your loved one, we encourage you to take a few moments to complete it. Your feedback is completely confidential and helps us continue to grow, improve, and honor our commitment to the families we serve.
We are deeply grateful to all who have shared their experiences. Thank you for allowing us to walk beside you.
